Dear Ambassador Dennis Francis, President of the 78th UN General Assembly, and UN Member States,
We, the undersigned collection of organizations and initiatives dedicated to advancing self-care and accelerating progress towards Universal Health Coverage (UHC), call on the drafters and Member States to explicitly and directly acknowledge and support the vital role of self-care in achieving UHC in the political declaration that will follow the UN High-Level Meeting on UHC in September 2023.
Self-care – the ability of individuals, families, and communities to manage their health and well-being with or without the support of a healthcare provider – is an undeniable aspect of individual health behavior and an important and growing component of health systems. In the Guideline on Self-care Interventions for Health and Well-being, 2022 revision, the World Health Organization (WHO) “recommends self-care interventions for every country and economic setting as critical components on the path to reaching UHC, promoting health, keeping the world safe and serving the vulnerable.” We know self-care benefits people and systems in many ways:
- Self-care does not replace the health system but equips people with more options to access the health system. It is especially important for vulnerable populations, including women, girls, transgender individuals, adolescents and young people, rural populations, those in humanitarian situations, and stigmatized groups such as people living with HIV and sex workers.
- Utilizing self-care to its potential can be a powerful means to promote gender-based autonomy by putting care in the hands of women and girls.
- Effective practice of self-care creates a stronger and more efficient health system, making good use of limited resources and alleviating pressure on healthcare workers by freeing them up for the most critical tasks.
Yet, self-care is an underrecognized and underutilized approach to health system strengthening. Many proven interventions – from self-diagnostics to self-managed contraception and digital solutions – remain out of reach for those who need them most. Only through the adoption of proper policy, regulatory and funding frameworks can we guarantee access to quality, evidence-based, effective, and affordable self-care interventions for all.
As the global community comes together to achieve the ambitious goals of Universal Health Coverage, we must use every lever available to increase access to needed health services, information, tools, and technology to drive better outcomes and help stressed healthcare systems. Self-care supports the achievement of all eight recommendations identified in the UHC2030 Action Agenda and should be mainstreamed within UHC frameworks and activities at the global and national levels. To that end, we call on the statement drafters and the Member States at the UN High-Level Meeting on UHC to:
- Explicitly and directly acknowledge and support the vital role of self-care in achieving UHC in the political declaration.
- Establish national policies and programming integrating self-care interventions as a component of national health systems.
- Adopt the new WHO Guideline on self-care, calling for national policies to usher in this transformative era in healthcare and support the inclusion of self-care in UHC.
Self-care can improve the health and well-being of individuals, whereby they exercise greater autonomy, power, and control of their health. At the same time, self-care can contribute to progress toward UHC by making sure health systems are more equitable and efficient. It is an under-used and high-potential way to help effectively achieve UHC, strengthen primary healthcare, and reach other health goals. We urge global leaders to take the clear and decisive actions outlined above to support and promote self-care.
