New estimates show that approximately 24% (218 million) of women and girls of reproductive age (15-49) in LMICs have an unmet need for modern contraception, and infrequent sex is the largest driver of contraceptive non-use among women of reproductive age in LMICs. On average, 34% of women in Latin America and the Caribbean, 31% in Asia, and 19% in Africa say they are not using a method because they have sex infrequently or not at all. Studies assessing women’s interest in using oral drug preparation in a coitus-dependent manner as a pregnancy prevention method suggest that desire for an on-demand oral contraceptive may be widespread.
PSI led demand forecasting and health impact modeling to understand the potential market size of an on-demand pericoital pill (ODP) in 129 low- and middle-income countries to inform investment decisions for potential introduction.
This work reveals that introduction of an on-demand pill has the potential to transform SRH outcomes by reaching a large proportion of women who have infrequent sex and may be interested in a woman-controlled, on-demand option that offers several benefits over existing products on the market.
Results
- Users: The model estimates that 49 million women of reproductive age may desire/prefer to use an on-demand pill by 2030. Over half (56%) of these women will be those who have not used a method of contraception before, followed by users of pills (14%), injectables (13%), implants (7%) and condoms (5%). We anticipate there could be 7 million users in the same period, accounting for access barriers across public and private sector channels. Most users will be in Africa (55%), India (18%), and South-Eastern Asia (10%), and about 60% of users will access the product through the private sector.
- Market size: The market size for the ODP is estimated to reach ~136 million doses by 2030, and the introduction of ODP is not expected to significantly distort the emergency contraceptive pill market. This seems reasonable given over half (56%) of the demand will be from new FP users.
- Impact: At scale, it’s estimated that there will be 40 million users reached annually, averting half a million unintended pregnancies and over 120,000 unsafe abortions each year
Whereas the time to introduction may change, largely depending on completion of clinical trials and fulfillment of evidence requirements for introduction, we believe the approach will stay consistent and model inputs and assumptions will be routinely updated as more information becomes available to ensure that it’s fit for purpose.
