PSI and the Global Fund to Fight AIDS, Tuberculosis & Malaria

PSI’s Organizational Strengths and Capacity
Globally, PSI has considerable experience with the implementation of Global Fund-financed programs in all three disease areas as a Principal Recipient (PR) or Sub-recipient (SR) in 40 countries and 84 grants over the last six years.  PSI also serves as a member of 14 Country Coordinating Mechanisms (CCMs) and is a technical advisor for several others.  As an implementing partner, PSI offers technical support to countries in program management, financial management, monitoring and evaluation, procurement, and contracts.  PSI also provides support to countries during the grant application process.

PSI as a Principal Recipient for Global Fund Grants
Through the Global Fund’s first six funding rounds, PSI and its local affiliates have served as PR for seven grants in five countries: Madagascar, Nepal, Nigeria, Tanzania and Togo.  In Round 7, PSI in Southern Sudan was confirmed as a first-time PR and in Madagascar and Nepal as a repeat PR with two new grants.  In addition, PSI was included as PR in eight Round 8 proposals. Five grants have reached the Global Fund’s Phase 2 review process (an in-depth performance review after 20 months to determine whether grant funding will continue for a further three years), and all five grants received unconditional renewals with “A” ratings.  The Global Fund has recognized PSI’s above-average performance as a PR and the success of its programs by featuring PSI programs in numerous Global Fund publications and on its website.

PSI as a Sub-recipient for Global Fund Grants
Through Round 6, PSI has served as the SR for a total of 61 grants in 31 countries and two regions (multi-country grants in the Caribbean and Central America): 30 malaria grants, 24 HIV grants, and 5 TB grants.  PSI currently has 31 active grants in 18 countries. The scope of these activities varies considerably, from small awards of less than $50,000 to large awards of up to $13 million.

PSI and Technical Support
PSI serves on the CCMs of 14 countries and sits on the technical committees of several other CCMs. In this capacity, PSI provides technical assistance to CCMs on governance and management, and has helped several countries develop successful Global Fund proposals.

PSI’s Commitment to Community Systems Strengthening
A key to PSI’s success is its emphasis on strengthening institutional development in the countries where it works, including through the creation and continuous strengthening of PSI local affiliates.  PSI’s local affiliates and country program offices benefit from the combined strengths of their local staff and in-county presence as well as their membership in PSI’s growing global network.  Several local affiliates have such significant local ownership, capacity and autonomy that they only require technical support from PSI headquarters.  Society for Family Health in Nigeria is a PR for two Global Fund grants.

PSI and Procurement
PSI has significant procurement capacity and is highly experienced in managing every aspect of the international supply chain, including quality assurance.  From 2006 to 2007, PSI’s procurement of health products increased significantly from $34.7 million to $63.9 million, including big increases in bed net procurement (from 3.5 million bed nets in 2006 to 6.9 million in 2007). In addition, PSI has been identified by the Global Fund as consistently compliant with the cost reporting mechanism on their website.

Examples of PSI-Global Fund Success
Madagascar
PSI has used a two-pronged strategy to prevent malaria as PR in Madagascar. First, it has increased informed demand for insecticide-treated nets by implementing a national behavior change communication campaign. Second, it has increased the accessibility of these nets by distributing subsidized insecticide-treated nets to pregnant women and parents of children under five through other non-governmental organizations and through public sector clinics where appropriate. More than 1.1 million nets have been distributed by PSI in Madagascar to date. PSI/Madagascar has recently been awarded another malaria grant totaling $69 million for five years.

Nepal
In Nepal, PSI is a co-PR with the Ministry of Health for malaria.  Through this award, PSI/Nepal is responsible for a variety of activities including: implementing behavior change communication campaigns; training private health care providers; and procuring and distributing insecticide-treated nets through social marketing and targeted free distribution to the poor. In recognition of the success of this Round 2 grant, PSI Nepal will also serve as PR for Nepal’s Round 7 malaria grant for $25.8 million over five years.

Pakistan
PSI and its local affiliate, GreenStar, are a SR on Pakistan’s Round 3 TB grant.  Through this award, PSI is implementing a public-private mix (PPM) project to increase TB diagnosis and treatment in target areas throughout Pakistan.  These are the project results so far: 1,010 providers have been trained in TB diagnosis and treatment; 962 private providers have been enrolled in the network; 89 private laboratories have been accredited to perform quality sputum smear microscopy; 10,557 community-based treatment supporters have been trained; over 200,000 people have been reached through outreach activities; 9,151 TB patients have been registered (7,155 sputum smear positive) with a treatment success rate of 80%. Greenstar’s work has contributed to between 33-51% of the TB case detection rate in the target areas.

Togo
PSI is PR for Togo’s Round 4 HIV grant for $30.5 million over five years.  With these funds, PSI has worked closely with the Government of Togo to implement an HIV prevention education program that includes: distributing subsidized male and female condoms; training health care workers; implementing innovative multi-layer behavior change communication campaigns; HIV voluntary counseling and testing (VCT); and treating sexually transmitted infections.  PSI has achieved its targets, with 23,000 people completing counseling and testing, 2,250 people trained to deliver behavior change communications outreach, and over 9.5 million condoms sold.

Zimbabwe
In Zimbabwe, PSI is a SR for both malaria and HIV (Rounds 1 and 5).  PSI Zimbabwe’s malaria activities include the targeted distribution of insecticide treated nets and retreatment kits through social marketing and free distribution, as well as a nationwide behavior change communication campaign to promote arteminisin-based combination therapy (ACT) for children under 5.  PSI Zimbabwe’s HIV activities include TB/HIV, behavior change communication, and VCT.  PSI Zimbabwe also serves as an NGO representative on the CCM and provides technical advice in all 3 disease areas.
